NCQA Recognizes APLA Health as Patient-Centered Medical Home in LA County

The National Committee for Quality Assurance (NCQA) announced that APLA Health has received Patient-Centered Medical Home (PCMH) Recognition for its focus on comprehensive, coordinated care for its patients and clients.

“NCQA Patient-Centered Medical Home Recognition is the benchmark for defining high-quality care focused on the patients. It emphasizes care coordination, health information technology, and access,” said Craig E. Thompson, CEO of APLA Health. “This recognition shows that APLA Health as the resources, skills, and staff to be out in front in providing the best care for anyone who walks through our doors.”

PCMH recognition is an indicator that a health center is an advanced primary care practice and has undergone a transformation in quality and safety enhancements to establish a foundation for coordination of care across the care continuum. In addition to improved quality and safety, APLA Health offers extended hours, care management for patients with chronic illnesses, mental health integration and much more. This PCMH Recognition applies to all three APLA Health primary care service locations and is valid for one year.
