Home Health Program

APLA Health provides intensive case-management services for HIV+ clients who are in need of home-based services.

The goal of the Home Health Program is to promote independence while allowing the client to remain in the home setting for as long as possible.

Each client is assigned both a registered nurse case manager and social work case manager. This way, we ensure that all of a client’s complex needs will be addressed by a professional with the necessary specialized knowledge and expertise.

The program uses a team approach to case management. Our team provides medical case management, psychosocial services and attendant care to clients in their homes.

You and your case management team develop a comprehensive plan for your specific situation. Your registered nurse case manager can help arrange for limited in-home attendant care to assist with daily living activities and household management. You will also receive treatment advocacy with your primary care physician; medication information; and help maintaining your treatment regimen.

Your social work case manager can help you access various services, including in-home mental health counseling, legal services, housing assistance, benefits counseling and medical transportation.

SERVICES WE PROVIDE

Management of HIV-related Symptoms
The home health program can assist with management of illnesses or conditions directly caused by your HIV.

Assistance with Comorbid Health Conditions
People usually don’t have one medical condition and those with HIV often see other medical conditions, including heart disease and diabetes. We can assist with illnesses or conditions unrelated to HIV, but that may complicate your HIV treatment.

Medication management
We can provide assistance in understanding what your medications are for and how to stay organized with your pill regiment.

Mental health counseling
Our home health team can coordinate in-home therapy sessions that help address feelings of depression, anxiety, and isolation.

Benefits navigation
There are a lot of available programs at the local, state and federal level that you may qualify for. We can offer assistance with understanding the full scope of benefits available to you and guidance with application for those benefits.

Substance abuse counseling
APLA Health can offer non-judgmental and supportive interventions to help address any alcohol or drug use you feel you might need help with.

Enrollment

Enrollment in the Home Health Program is based on criteria established by the California Department of Health Services, Office of AIDS.

To be eligible to receive Home Health Service at APLA Health, you must:

  • Be diagnosed HIV+
  • Be a client of APLA Health
  • Participate in an assessment by a Home Health Program case management team
  • Be a resident of Los Angeles County

APLA Health’s mission is to achieve healthcare equity and promote well-being for the LGBT and other underserved communities and people living with and affected by HIV.

This program is funded in part through grants awarded by the California Department of Public Health, Office of AIDS and the County of Los Angeles Office of AIDS Programs and Policy.
